Hello wonderful parents! We know that being a parent is a non-stop adventure, and your to-do list can seem never-ending, but fear not! We're here to lend a helping hand and make life a little easier for you.


Today, we're diving into the world of immunization records – a crucial part of your child's health journey. But don't worry, we've got the key to unlock the information you need!

No more digging through piles of paperwork or running around trying to find those elusive vaccination records. We've compiled a stress-free guide to help you access your child's immunization records from the Oklahoma State Immunization Database, our patient portal, or even from another state. Fortunately, accessing and managing these records has become easier than ever! Let's get those records checked off your to-do list, hassle-free! 📋✨🤗


1. Obtaining Immunization Records from Oklahoma State Immunization Database:

The Oklahoma State Immunization Database is a centralized repository of immunization records for residents of the state. Here's how you can request your child's records from the database: Go to Oklahoma Immunization Database

*** Patients and their Legal Guardians can use this screen to view and print an official record of their immunizations. The patient's First Name, Last Name, Date of Birth, and Gender are required before proceeding.

If you are unable to obtain your child's shot record from the OSIIS network you can email them at OSIISHELP@health.OK.Gov, call your pediatrician's office or go your pediatrician's portal and request help.


2. Requesting Immunization Records from Our Patient Portal:

Simply log in to your Patient Portal using your email on file or head over to our website's 🔗"Patient Portal" section to access a wealth of convenient features! 🌐 Go to your documents section or under Medical records and then "IMMUNIZATIONS"

The Patient Portal opens up a world of possibilities, enabling you to request your child's immunization records. (Also order prescription refills, request an appointment, find lab results, referral status and message providers and staff.)

We are currently in the process of integrating all of your child's immunizations reported to the State of Oklahoma (OSIIS) to your Bright Future Pediatrics portal. Any immunizations from previous practices or completed in our office before March 1, 2023 may not be in our new system just yet. (Full integration is expected to be complete by October 1, 2023) If you go to the portal and do not see your child's records or if you think there is an error in your child's records just send us a message in the portal and we will review or update them for you in 2-3 business days. You can also stop by the office to get a printed copy for free if you need them the same day. *Please note: Due to new compliance rules we cannot fax or email them to you.


3. Requesting Immunization Records from Another State or Provider:

If your child received their shots in a state outside of Oklahoma or at another PCP and they are not documented in the Oklahoma State immunization registry you will need to call your child's previous healthcare provider and request that they send you your childs records. You can also request that they sent those records to us by completing the medical records request form and sending it to your previous PCP. Once they get it they should fax these to our office and we can print them for you or you can request that we update them for you in the portal within 2-3 days of request.
